Responsibilities:
1. Evaluate the patient’s medical intake forms to establish if they meet criteria for ketamine. 
2. If a patient meets criteria, the initial evaluation is conducted via telehealth at your convenience, documented in our medical record system.
3. Prescribe initial sublingual ketamine dose to be used by the patient in their therapist’s office.
4. Connect with the patient via phone call after their 1st ketamine session to inform urther dosing strategy.
5. Prescribe the remaining five doses of ketamine to the patient.
6. Be available via email or phone for any issues the patient may need help with.
7. Rarely patient needs Zofran called into local pharmacy. 
Requirements:
1. You must maintain an active NP license and DEA in the state where you will be prescribing to patients.
2. You must not have a history of professional discipline or loss of license.
3. You must be able to prescribe controlled substances and maintain all licenses necessary to do so. 
4. You must carry malpractice insurance. 
Compensation:
–$80/hr with the anticipation you will see two clients per hour (higher if you are doing in-home visits.)
